Navigation services provide assistance to under-served populations seeking to obtain needed health access and social services assistance. Through our Navigation, Outreach and Education programs State Certified Community Health Workers provide education, information and one-on-one assistance on health and community resources. We offer training and direct application assistance in enrollment for CHIP/Medicaid, and other federal and regional indigent financial assistance programs, with a focus on uninsured and under-insured populations. Additionally, Navigators educate clients on the importance of locating and using a health home to achieve and maintain good health and link them to other community resources. In 2015, Gateway to Care (GTC) navigated more than 44,000 clients to services.
