PATIENTS

The Provider Health Network (PHN) is a program designed to help the uninsured population of the Greater Houston Area access healthcare. If eligible, the patient is enrolled in the program for 6 months. Extensions may be granted to the patient if he or she still satisfies eligibility requirements. 

The PHN, however, is not a self-referral program. Patients can only be referred into the PHN by a participating physician or community clinic.   

Eligibility 

bullet150% of the Federal Poverty Level (see chart below)
bulletNo health insurance of any kind; cannot have a Gold Card either.
bulletIndividuals must be from the Greater Houston Area
bulletMust have a primary care home*

 * If a patient is not in one, the PHN will help direct individuals to appropriate primary care clinics.
