The Provider Health Network (PHN) is based on a national model called Project Access where a network of physicians and ancillary service providers have agreed to dedicate a portion of their practice for uninsured patients who are pre-screened and determined to be eligible for the program. Project Access is being practiced in 50 other cities across the nation (e.g. Dallas, Indianapolis). This model has had positive outcomes in ER utilization, healthcare costs/capita, and in increasing the number of patients who find primary health/medical homes. PROVIDER HEALTH NETWORK SERVICES* o Physicians – Primary and specialty physicians donating services. o Labs – LabCorp is providing lab services for the PHN volunteer physicians. o Diagnostic – We have agreements from different companies to provide diagnostic procedures. o Prescriptions – The PHN is able to provide a certain level of support for prescriptions. o Transportation – The PHN can provide transportation vouchers. o Patient Care Coordination – Provide one on one support to guide patients through the process, assist with the paperwork, translation, education, and general access to other resources as needed. The Provider Health Network is a very organized system that facilitates connecting healthcare providers in a very efficient and controlled manner. Patients cannot self-refer themselves into the program. They must be referred by a participating physician or one of the Collaborative member community clinics (make clinics a link to the clinic list). With a strong partnership from physicians, hospitals, and other healthcare providers, the Provider Health Network will truly be an effective force in the Greater Houston Area community.
